A parent or guardian must complete this form if their child will travel to or from a 4-H event by any method other than transportation organized by the County Extension Office.
- Examples include:
  - 4-Her will be riding with a parent/guardian.
  - 4-H’er will be driving him/herself.
  - 4-H’er will be riding with an adult who is not the parent or guardian.
  - 4-H’er will be riding with another 4-H’er.
  - 4-H’er will be traveling by public transit or rideshare
  - 4-H’er will be arriving late or departing early from the event
This form must be filled out in full, signed, and submitted before the event.
After submission, a copy of the completed form will be sent to the event coordinator, the County Extension Office, and the parent or guardian.
