Mathematics Autobiography Survey
Dear Participant,
My name is Susan Cannon, and I am a faculty member in the Mathematics Science and Social Studies Department at the University of Georgia. I, along with colleagues, Stephan Caviness, Aida Alibek, Saniye Ergan, Kelly Dollarhide, and Luke Zhuang, am inviting you to take part in a research study.
We are doing research on how math teacher educators use mathematics autobiographies to inform their practice and or to engage teachers with their own identities.
You are being invited because you are a mathematics teacher educator who uses mathematics autobiographies with their students.
If you agree to take part in this study, you will be asked to take a 10-15 minute survey.
Participation is voluntary. You can refuse to take part or stop at any time without penalty.
There are questions that may make you uncomfortable. You can skip these questions if you do not wish to answer them. Y
our responses may help us understand how mathematics autobiographies inform mathematics teacher educators' practice and support current and future mathematics educators.
We will not collect any information that would or could be linked to you personally in the survey. This research involves the transmission of data over the Internet. Every reasonable effort has been taken to ensure the effective use of available technology; however, confidentiality during online communication cannot be guaranteed. Due to the anonymous data collection technique, withdrawal of data is only possible before responses are submitted. Once you submit your responses, your data cannot be removed.
